Provisions Applicable to All Plans



Living Benefit
This feature allows you to receive 50% of your death benefit (amount of coverage) to help with your care should you contract a terminal illness. To receive the benefit, you must be under age 69 (under age 84 - age 79 for NY residents - if insured under the Lean 15 Plan), have been diagnosed by a physician, and given a life expectancy of 12 months or less. The benefit is paid only once and the future death benefit payment is reduced by such amount. Premiums are not reduced. The rest of your benefit would be paid to your beneficiaries at the time of your death.

Note:
  • Receipt of Living benefits may affect eligibility for public assistance programs and may be taxable. Prior to applying for Living Benefits,     insured should consult with appropriate social services agency and a qualified tax advisor.

  • Living Benefits are not available to residents of Massachusetts.

  • The Living Benefit is payable, between age 50-69 under the Level to age 50 and Militia Plans, will be based on the reduced coverage      amount then in force - see plan descriptions for details.)

Effective Date
All coverage is subject to underwriting approval by New York Life. Approved coverage will be effective on the date specified, provided the premium has been paid. Members/Spouses must also be performing their normal activities on the date of approval. Dependent children insured under Children’s Coverage must not be hospital-confined on the date of approval. Payment of premium does not mean coverage is in force before the effective date specified by New York Life.

Incontestability
The validity of any amount of insurance, which has been in force for two years (one year under the Lean~15 Plan) during the insured’s life, will not be contested except for eligibility and nonpayment of premium contributions.

Beneficiary
Members may name anyone they choose as the beneficiary for coverage on their life. Members may also change their beneficiary at any time by written request or add other beneficiaries as needed. The beneficiary of dependent coverage shall be the insured member.

Overall Plan Maximum
The aggregate maximum for which an individual may be covered under all life insurance coverage issued through USBA is $650,000.

Conversion Privilege
These Plans provide the opportunity to convert to an individual policy without taking a medical exam. This privilege exists when coverage terminates subject to certain conditions that are described in your Certificate.

How New York Life Underwrites Your Request for Term Life Insurance Coverage
Information regarding insurability will be treated as confidential. In considering whether the persons in your request for insurance qualify for coverage, we will rely on the information you furnish, and on any information we may ask you to obtain from your doctor, other medical practitioners and facilities, etc. New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law. This information may be seen by New York Life, Militia administrative services, and USBA employees, but only on a “need to know” basis in considering your request. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved. If we cannot provide the coverage you request, we will tell you why. If you feel our information is inaccurate, you will be given a chance to correct or complete the information in your files. Generally, upon written request, medical record information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured. If we can provide the coverage you requested, we will inform you as to when such coverage will be effective. Under no circumstances will coverage be effective prior to this date. Payment of a premium contribution with your application does not mean that there is any insurance in force before the effective date as determined by New York Life.
